Excellence in Care

Warm drink at home - Oakwell Homecare

At Oakwell Homecare we have a committed team of professionals who are dedicated to meeting your care goals. Our care support staff are devoted to ensuring older people, and people with other non-age related care needs, are supported to remain healthy and independent for longer. We strongly believe in delivering high-quality care and support at a time and place that’s appropriate for you and your loved ones.

We pride ourselves in delivering exceptional standards of care, including meeting the complex care needs of those with cognitive impairment and dementia. Our team of highly trained carers are equipped with the knowledge and skills to identify changes early (this may include changes in behaviour or mood, immobility, medication queries, dehydration and poor nutrition).

By identifying these changes in a timely manner, we can facilitate better outcomes for every individual using our service.

Across the UK, health and social care services are sadly failing older and more vulnerable people. The NHS is struggling to deal with increasingly difficult winters, and has not properly restored capacity since the Covid pandemic. Many of the issues affecting the NHS and social care are long-standing, including years of under-investment and failure by authorities to reform services.

At Oakwell Homecare, we’re committed to delivering outstanding care solutions. We believe our comprehensive initial assessment of your needs results in a personalised care plan that prioritises and implements interventions in a coordinated way. The team manages your care needs in a balanced, holistic manner, particularly where treatment of one condition may worsen another.

Enabling independence, respecting autonomy, promoting shared decision-making and anticipatory care planning are the key tenets of our service to you.

Sit Up, Get Dressed and Keep Moving!

Why is putting in place good quality care for yourself or your loved one so important?

Well, it prevents frailty, a condition which:

  • limits life,

  • increases frailty-related disability,

  • increases dependency,

  • has a negative impact on quality of life, including morbidity and mortality,

  • causes further escalation of acute and long-term health issues,

  • and can escalate your care costs in future.

Receiving the right care means frailty is potentially reversible, especially in its earlier stages, and can be managed well by comprehensive assessment and tailored interventions, care and support.

We believe in engaging and empowering individuals, and giving confidence back to each person by restoring their autonomy and independence.

Our pledge is to actively listen to our service users, allowing us to ensure a personalised approach to care, based on shared decision-making guided by your wishes and your priorities for your own care.

At Oakwell Homecare, we keep ourselves aware of what’s important to you. We work collaboratively with you and your family to achieve these critical aims:

  • Staying in your own home for as long as you can, with the correct support to help you achieve your goals.

  • To age well and live well at home for longer.

  • Staying independent – being able to undertake daily and social activities with confidence, enabling independence and promoting wellbeing.

  • We’re strong advocates of the BGS ‘Sit Up, Get Dressed and Keep Moving!’ campaign to prevent deconditioning.

Ultimately, we believe wellbeing encompasses physical, mental, emotional and spiritual health.

Loneliness and social isolation are associated with increased frailty and physical disability.

Our care staff are trained to look out for signs of decline in all these areas, so that you can be supported and signposted towards the care and interventions that promote your overall wellness and life quality, not just fulfil the most basic needs.
